As we begin to slowly reconvene post-COVID, we are doing all we can to provide a safe environment for our church family. We are being asked to roster all attendees at our worship service for use of potential contact tracing in the event of a confirmed case within our population. Therefore, we must ask that each family household complete this form each time they attend worship service. We also ask that you and your family members do not attend worship service if you are exhibiting any signs of COVID (fever, cough, shortness of breath, muscle aches, sore throat, headache, nausea, congestion, runny nose, loss of taste or smell or diarrhea). Please also do not attend if you or any family members have had direct contact with someone currently testing positive for COVID.
